Several people have asked how to prepare datasets for NLME. I thought it would be a good idea to open a thread for this topic, because it depends on the format of the original dataset(s) and the model you want to fit. So, if anyone wants to share sample datasets or let us know what format your original data is in, we can tell you how to prepare it for NLME.
